Credit Report Request Window Field Definitions

First Name

Enter the first name of the person for whom you are requesting a credit report.

Last Name

Enter the last name of the person for whom you are requesting a credit report.

Middle Name

Enter the middle name of the person for whom you are requesting a credit report.

Suffix

Enter the name suffix of the person for whom you are requesting a credit report (for example, Jr., Sr., II, III, and so on.)

SSN

Enter the 9-digit social security number of the person for whom you are requesting a credit report.

Date of Birth

Enter the birth date of the person for whom you are requesting a credit report.

Gender

Select the gender of the person for whom you are requesting a credit report from the drop-down list.

Telephone Num

Enter the telephone number of the person for whom you are requesting a credit report.

Access PIN

Enter the PIN provided by the person for whom you are requesting a credit report.

Note:

If a person blocks on-line access to his/her credit information, they must provide you with their PIN in order for you to obtain a credit report for them.

Access PIN Bureau

Select the credit bureau that provided the Access PIN from the drop-down list.

Note:

This field is required, if Access PIN is entered.

Address Information

House Number

Enter the person's house/street number.

Directional Prefix

Select the person's street prefix (for example, North, South, East, West, and so on.) from the drop-down list.

Street Name

Enter the person's street name.

Street Type

Select the person's street type from the drop-down list (for example, Street, Road, Lane, Parkway, and so on.).

Apartment Num

Enter the person's apartment number, if applicable.

City / State / Zip

Enter the person's city, state, and zip code.

Note:

You must enter a City, State, and Zip, if you enter any of the above address information. The last 4 digits of zip code are optional.
