Create Ported-In Service Number

When you select Create Ported-In Service Number, the Port-In Service Number window is displayed.

1. Enter the required information described below. Mandatory are marked with the symbol; other fields are optional.

   Service Number (Required)	Service number to be ported
   Carrier Porting From	Select name of carrier from drop down list
   Time Zone	Select the time zone of the location where the number is being ported from. (also mandatory)
   User Name	User requesting and authorizing the port-in
   Account Number	Complete at least one of these fields.
   SSN
   Tax ID
   Password/PIN	Password or PIN for the existing account with the original carrier.
   Billing Information
   Name	First, last and middle initial or company name
   Prefix/Suffix	A prefix or suffix of the name
   Company Name	Company name if applicable
   Street Number (Required)	Street address
   Street Direction	Street direction if applicable
   Street Name	Street name
   City / State / Zip	City, State and ZIP Code
   Country	Country name

2. Click OK to continue.
