This only applies and is only available when your system is set up to use this functionality.
The NLAD tab is used to enter and maintain Lifeline eligibility information about a Lifeline customer to be coordinated with the National Lifeline Accountability Database. If your company is not an eligible telecommunications carrier (ETC), you do not need to maintain this information and this tab is not be displayed.
Once all the appropriate fields have been updated, click Check Subscriber to verify the subscriber's eligibility.
The NLAD tab provides the following information:
Section | Description |
Independent Household | Independent Household Flag – Select if subscriber is an independent economic entity sharing an address with another subscriber (as determined by completing the Multi-household Worksheet). Certification - Date subscriber completed and signed the worksheet. This field is required if the IEH check box is checked. For a customer who was a subscriber prior to June 1, 2012, and the date is not available, enter: 05/31/2012. Recertification - Date IEH information was re-certified. |
Primary Address | Temporary - Check if primary address is a temporary location. These flags are mutually exclusive: Tribal Primary Address – Select if address is in Tribal lands and not registered with USPS address matching service (AMS). Rural Primary Address – Select if primary address is in a rural area, and not registered with AMS, nor able to receive postal delivery. Address Confidentiality Program – Select if subscriber is participating in Address Confidentiality Program (ACP). No Additional Flags – None of the above selected by default.

Benefit Qualifying Person | These fields are only required when the Benefit Qualifying Person is someone other than the subscriber. BQP same as Subscriber - Checked by default and associated fields disabled. If Benefit Qualifying Person is someone other than subscriber, uncheck this checkbox to enable the BQP fields. When enabled, all fields and either Last 4 of SSN or Tribal ID are required. BQP Last Name BQP First name BQP Middle name BQP Date of Birth BQP Last 4 of SSN - Required if Tribal ID is not provided. BQP Tribal ID - Required if SSN is not provided.

TPIV Resolution Required | When a TPIV error occurs during Lifeline enrollment, users are required to check documentation to confirm the subscriber’s identity. When TPIV Resolution Required is checked, these related fields are enabled: Agent Info – Click Edit to display Agent Info form with the following: Agent Name - autopopulated with the name of the logged in user. Agent ID - autopopulated with the location (store) ID. Agent DOB - not a required and can be left blank Way to Reach - Click Edit to open a dialog that lets you select e-mail, phone, text message, or mail. This is used to populate the Lifeline application form and is required. Note: E-mail is only available if the customer has an e-mail associated with their service. Certification Check Box – Check to certify that you reviewed the selected documents. These fields are typically defined in POS make Sale. The main purpose of the TPIV Resolution Required check box in Customer Management is to remove the unresolved TPIV error status on a customer once the error is resolved. This action is permission-based.
The submission job that sends updates to NLAD ignores (does not auto-enroll or send updates for) customers with a status of unresolved TPIV error, except for de-enroll transactions. Authorized users need to uncheck this check box once the error has been resolved.
