This action is installed by the Orders web module. It creates a Disconnect order for a service using either the Service Number ID (UsgSvcID) or Port Request ID. For example, this Action can be paired with the Port Request Modified Trigger to automatically disconnect a Service upon completion of a port out request.
PortRequestID | This is optional; however, either this field or ServiceNumberID must be populated. |

Disconnect Reason | This is required. |
DisconnectDate | This is required if using ServiceNumberID; otherwise this will be derived from the port-out request's create date. |
